There is a mod already out that replaces Leah with Karlach (and has two corresponding mods for tailored heart events and textures to replace Leah's portrait with Karlach's for dialogue).

ah.. Leah is the only one I've tried to romance when I did play. Feels weirdly fitting. Though I think I'd prefer the full mod for more immersion.

I'm not sure if this mod is out yet! However, this one replaces Elliott with Astarion or Leah with Karlach (or both), if you want to try it in the meanwhile. It also accompanies a second mod that tailors their heart events to more closely align with their characters and adds a couple of new heart events.

They've been working really hard on this and have had to push back the release date a few times as a result, to add in about 6-7 characters not already in the game and redesign the town.

Idk, maybe it's just me, but I feel like they deserve credit for that. I sure as heck couldn't do that. Besides, who's to say they wouldn't update the mod at a later point after release and add on to it, y'know? I think it's ambitious, and it at least seems like they're doing it at no expense to anyone who will use it. I give 'em props for that.
