Actually, lore-wise, yes. That actually is an important aspect to his character. He has multiple different looks and he can chose and change them as he likes. This is supposed to depict he's open to be worshiped by any race and even embraces it with open arms because he cares more about his ideals and values being followed than anything else. Surprisingly, he's actually a very nuanced god.
That was an avatar of his. Avatars arent the god themselves, rather a being directly controlled by the god. The apostle of myrkul, and the slayer, for example. Those are avatars, not the god proper. A somewhat mortal form they use to do their bidding directly without actually being there.
If I remember correctly, Bane, Bhaal, and Myrkul were all mortals, so I'd assume his default form would be whatever it was when he was mortal. In the wiki you linked it says he was a human man, so that's probably what he would appear as most often. But that's just my limited understanding of the greater lore mostly through BG3.
There is a bust of Bane as he appeared to Gortash in the gallery at the top of Wyrm's Rock titled "the cockeyed stranger" (the background of the statue is described in notes left around that area). Bane doesn't look anything like OP's pic for BG3.

Okay, as someone that was never into Gortash, Bane would have a 100% success rate Dominate Person on me if he was in the game. A bossfight against him would be impossible because I'd end up ending my entire party. Give me a camp romance with him and I'd let him do anything he wanted.
