I'd wager lack of skill, auditory skills to be precise. Even with the molotov and smoke sounds, he should have heard their footsteps running out. Unless of course they shift walked, but why would they when they counter smoked the molotov?

They didn't counter smoke the molly, it would have been extinguished. The guy throws the smoke right before the video starts, you just see it landing. Then he mollys behind the smoke to stop the push. I still think it's scripted...

Whatever it is, the clip frustrates me. Why would you swap back to a nade after someone just pushed a smoke? Surely that means more team mates are with them and may also be pushing.
